About Jason Hoke, MD
Dr. Hoke began his medical career in Oxford in 2001, when he and two partners, Drs. Nathan Morris and Michael Fain, created Indian Creek Family Health Center. Over the course of the next 10 years, their practice grew from these three providers to eleven providers caring for patients at four different offices. In 2014, Indian Creek joined Kettering Health Network, and continues to be a part of this strong network of physicians. Over the next several years, Dr. Hoke felt a strong calling to return to the freedom of owning an independent medical practice, where he could ensure the needs of the patient were always the top priority. From the beginning of his time in Oxford, Dr. Hoke has been passionate about delivering high quality, all-encompassing care at an affordable cost. With the creation of Hoke Direct Primary Care, he will have the opportunity to fully embrace this calling he has for healthcare delivery.
Jason and Jennifer, his beautiful (and, oh so patient) wife of over 20 years, have six children, whom they have had the awesome opportunity to homeschool. Their two oldest boys are active in football and their youngest is a beast on the soccer pitch. Their three girls are enjoy dance, gymnastics, and horse riding. The family is active in the choir and many other activities at their church in Camden, Ohio. In their free time, they enjoy spending time outdoors, cooking together, relaxing with a good movie, or cheering for the Bengals -- Who Dey! They are proud to call Oxford home, and enjoy many of the amenities this college town has to offer.

About Julie Greene, APRN
Julie began her nursing career in Oxford in 2004 as an LPN at Indian Creek Family Health Center working with Dr. Hoke. She loved the primary care setting, but desired to gain more experience in a higher acuity setting, so she moved to Reid Hospital. In 2010 she completed her training to be a Registered Nurse. She then joined TriHealth, where she worked for the next 10 years gaining experience as a charge nurse and an assistant nurse manager. Despite the love of her career in the hospital setting, she had a strong desire to become a nurse practitioner and return to the primary care setting. In 2021, she completed her Masters of Science in Nursing at the University of Cincinnati and has worked gaining valuable experience as a family nurse practitioner ever since. Now, coming back full circle to the start of her career, she will be working with her mentor, Dr. Hoke, yet again. She is excited about practicing in a Direct Primary Care office, where she can develop long-term relationships with her patient’s and provide high-quality, personalized care.
Julie was born and raised in Cincinnati and has resided within Butler County for the past 28 years. She is married to her high school prom date, Justin, who she shares 2 children and 2 fur babies with. Her son is active in football and wrestling while her daughter is active in cheer. They have two German Short Haired Pointers that are high energy and love to run with the family. Together they enjoy spending time in the outdoors, biking, fishing, and family trips to the lake or the beach. She is active in her community and children’s sports as a youth coach and board member.

About Kara Hornung, RN
Kara entered the medical field in 2007 as a nurse's aide working at McCullough Hyde Memorial Hospital. While working there, she attended Ivy Tech Community College, graduating in 2010 as a Registered Nurse. In nearly ten years at the hospital, she grew into a multifaceted nurse. She worked on the Inpatient Care Unit, the Intensive Care Unit, and then transferred to the Emergency Department, where she spent the greater part of her career as an ACLS (Advanced Cardiovascular Life Support) Charge Nurse. She made compassion a priority and vowed to always see patients as people, not numbers. In 2015, she escaped the hectic hospital setting and took a position at the office of Good Medicine. She enjoyed the calm atmosphere of the office setting, allowing relationships to form with patients and coworkers. She states, “It is always nice to see familiar faces, and get to know new ones.” We are excited to have her as an integral part of our team!
Outside the office, you can find her with her best friend, her husband of nine years. She is also a notorious animal lover, and could not possibly imagine life without her “fur-babies”. She also enjoys spending time with her beloved family and friends, traveling, fitness, fishing the lake with her husband, and tackling home improvement projects!
About Debbie Shrader, RN
Debbie grew up in Oxford, Ohio and graduated with Honors with a Bachelor of Science in Nursing from the College of Mount St. Joseph (Cincinnati) in 1990. Her first post-graduate job was at McCullough-Hyde Memorial Hospital as a Medical/Surgical Nurse. She then spent 10 years moving around the country with her husband, Steve, who was on Active Duty in the Air Force. Debbie continued to serve patient needs in various areas: as a Cardiac and Medical/Surgical Care nurse in New Jersey, as a Diabetic Educator in Oklahoma, and as a Nursing Home Patient Care Coordinator in Mississippi. After returning to Ohio, Debbie taught prevention and nutrition in a local elementary school. Most recently, she worked as a nurse in Allergy and Asthma.
She and Steve have two beautiful daughters, Adelle and Rebekah, who are now both grown and newlyweds to wonderful young men. When the girls were still at home, the family was very active in an Oxford homeschool Co-Op. Outside of work, Debbie can be found either at church or at home walking her three canines – two German Shepard puppies and an Anatolian Shepherd -- on their farmland. Debbie is heavily involved in the Children’s Ministries at her local church. Her hobbies include reading, walking/hiking/camping, 4-wheeling excursions, and spending time with her family and friends. Debbie is grateful for the opportunity to work alongside Dr. Hoke and to have the freedom to put patient care first.
About Michelle Schomaker
Michelle was raised in Fairfield, Ohio and went to Cincinnati State where she studied Occupational Therapy Assisting. During her studies, she worked in a Physical Therapy Clinic as an aide. Shortly thereafter, she was married and her focus shifted to her new role as a stay at home mother. Michelle and her husband have three children, with their eldest son already having graduated from Talawanda and pursuing firefighter training. Michelle has been intimately involved with homeschooling their other two children for the last 4 years. Her younger son is active in the Talawanda marching band, and her daughter is involved in soccer. As the children have gotten older, Michelle has again felt the call to work outside of the home. She has worked in the community for Talawanda Schools and Miami University, but has always maintained an interest in the medical field. She was very excited to join the Hoke DPC team as an office receptionist/assistant.
In her spare time, she enjoys spending time with her husband, Brian, and the rest of the family. She enjoys helping him in the garden and 4-H activities with their kids. She teaches at their homeschool co-op and is involved with the teenage ministry at church. She likes decorating, being crafty, knitting, and refurbishing old furniture
About Taylor Jarvi, RN
Taylor grew up local to the Oxford area graduating Talawanda High School and then from The Christ College of Nursing and Health Sciences with her bachelor’s degree in nursing. She started her nursing career at Mercy West Hospital in 2014 on a medical-surgical/telemetry unit but found herself working closer to home in 2017 at McCullough-Hyde Memorial Hospital. There she worked on the Inpatient Care Unit as a charge nurse and obtained her certification in PALS (Pediatric Advanced Life Support).
When Taylor is not working you can find her spending time with her husband, Michael, of 10 years and her two boys, Kohen and Jace. They have a love for the outdoors whether that be going to the barn to feed the animals, playing in the creek, camping, boating, or swimming in the pool, they love doing it together as a family. Taylor also enjoys cooking, baking, collecting house plants, and a good DIY project.
