Direct Primary Care (DPC)

Direct Primary Care (DPC) is a model of health care delivery in which there is a direct, personal yet professional relationship between the primary care provider and his/her patient, unbiased by any outside contractual influences. Essentially, it is medicine in its purest form.

What does that mean for you? It means you get all encompassing, comprehensive primary care at an affordable price! Did you know that 80-90% of the health issues you may experience can be managed by a family physician? At Hoke Direct Primary Care, you will have access to all the services you would expect in a full-service family practice office, plus a few you would not expect:

Significantly discounted laboratory testing, only available to members
We draw blood in the office, and charge a fraction of what other facilities do for the same testing
Membership Loyalty Bonus: After one year of membership, and every year of continued membership thereafter, all adult members will receive an EKG and routine laboratory testing (CBC, Comprehensive Metabolic Panel, TSH, and lipid panel) at 50% off, for their preventive/wellness visit.

In-office prescription medication dispensing, only available to members
We purchase medications directly from a wholesaler at a deeply discounted rate, and pass the savings on to you, the patient
Rx cost savings of up to 80% off the pharmacy cash pay price.

In-office pharmacy

Routine preventive/wellness exams from birth to age 100+
Routine women’s health exams
Chronic disease management (diabetes, COPD, hypertension, heart disease, depression, asthma, etc.)

Virtually 24/7 access to your provider
Same Day and Next Day appointments for routine issues that need evaluated in the office, during normal business hours
Initial visits are scheduled for 60 minutes, follow up chronic care visits for 30 minutes. Acute visits scheduled for time based on acuity/severity of the need.
A limited panel size means more attentive, individualized care. Most traditional full-time physicians carry a panel size of >2,000 patients; we will enroll less than 1/2 that number.
After hours, cost saving care: avoid expensive Urgent Care visits by calling us first, as most issues can be handled by us.
Think you broke a bone? Call us and we can arrange for an Xray and arrange for fracture care
Have a bad laceration? Call us and we can do the stitches
