Now accepting new patients!
OUR VISION
Family Medicine is built upon a personal relationship between a patient and their provider. The physician or nurse practitioner knows the patient well, fully evaluates the health concerns, and discusses options for treatment to bring about healing and maintain wellness.
Healthcare Reform
As managed care grew in its influence, physicians and nurse practitioners noticed they had less time to focus on managing the clinical needs of their patients. Discussion of treatment options were replaced by protocols and algorithms. Personalized care was replaced by population health guidelines. And unfortunately, the needs of the patient competed with the needs of the payer, be it an insurer or Medicare.
Patients Come First
There is a growing group of physicians and nurse practitioners who desire to get back to the heart of family medicine, where the needs of the patient always come first. They are rediscovering the joy of practicing medicine without being encumbered by the restrictions of insurers or Medicare. They have joined a healthcare revolution, practicing a style of medicine known as Direct Primary Care. And the patients are reaping the benefits of this undistracted care.
Join us, and discover what it means to experience Medicine … simplified.

What is DPC?
Direct Primary Care (DPC)
Direct Primary Care (DPC) is a model of health care delivery in which there is a direct, personal yet professional relationship between the primary care provider and his/her patient, unbiased by any outside contractual influences. Essentially, it is medicine in its purest form.
What does that mean for you? It means you get all encompassing, comprehensive primary care at an affordable price! Did you know that 80-90% of the health issues you may experience can be managed by a family physician? At Hoke Direct Primary Care, you will have access to all the services you would expect in a full-service family practice office, plus a few you would not expect:
-
Significantly discounted laboratory testing, only available to members
-
We draw blood in the office, and charge a fraction of what other facilities do for the same testing
-
Membership Loyalty Bonus: After one year of membership, and every year of continued membership thereafter, all adult members will receive an EKG and routine laboratory testing (CBC, Comprehensive Metabolic Panel, TSH, and lipid panel) at 50% off, for their preventive/wellness visit.
-
In-office prescription medication dispensing, only available to members
-
We purchase medications directly from a wholesaler at a deeply discounted rate, and pass the savings on to you, the patient

Telemedicine
Not every health issue needs to be evaluated in the office. Communication, diagnosis, and treatment recommendations can sometimes be given via telephone, text, email, or video-conferencing. Welcome to 21st Century medicine!
"After going to this model of healthcare with Dr. Hoke, we will never go to a traditional physician plan. The care and time he can give is unprecedented. We never feel rushed and feel so valued as patients... The prescription prices have saved us the cost of his fee alone and Dr. Hoke always works with us to get the best medicine at the best price. We consider him a family member because he truly treats us as such."
